The information gathered by Horizon Health generally falls into two categories: (1) information supplied by visitors to the site when they register or initiate transactions, and (2) tracking information gathered as visitors navigate through the HorizonHealth.com site. Third-party providers, whose sites are featured on and accessed via our site, may gather information as well.
The information you provide us helps us to respond to your expression of interest in employment. It also assists us in matching you with available employment or physician practice opportunities.
When you contact us about employment or physician practice opportunities, we require some types of information, and provide you with an election to provide other relevant information:
When you apply on line for a particular job opening, you may be required to supply us with the following personal information; your:
Primary telephone number
Desired job status and shift; and, information concerning whether you have ever been employed at, or applied for a position with, one of our facilities before.
You may elect to provide us with additional personal information such as your e-mail address and salary requirements.
If you register to create a Physician Profile we require that you provide us with;
Your name; and,
You may elect to provide us with additional information such as information regarding your desired position, preferred location, your skill and educational levels, and your employment history.
Whether applying on line for a particular job or creating a Physician Profile, you may supply us with additional personal information if you elect to voluntarily send us your resume, or information contained in e-mail messages.
In many cases we will respond to you via the internet, but the contact information you provide may also be used to get in touch with you through the mail or by telephone. We may also use this contact information to provide you with updates regarding job or physician practice opportunities.
The information we collect from our visitors is stored in databases we control. Physical access to the servers on which we store visitor information is restricted. We don’t share your personal information with anyone, except as may be necessary to verify your education, experience, and other qualifications for employment. To protect the privacy of your personal information, we encrypt this information for transit over the Internet using a Secure Sockets Layer (“SSL”) connection. However, information you submit through use of your own e-mail program is not secure. As effective as modern security practices are, no physical or electronic security system is impenetrable. We cannot guarantee the security of our database, nor can we guarantee that information you supply won’t be intercepted while being transmitted over the Internet.
Most Web browsers allow use of an SSL connection. If your browser does not support use of an SSL connection, or if you prefer not to send your personal information over the Internet, you can mail it to the appropriate address noted on the Contact Horizon Health Employment page of this web site.
Retention of Information
All of the information that job applicants or physicians inquiring about practice opportunities provide through HorizonHealth.com is retained and destroyed in accordance with established record retention policy, which covers information collected through HorizonHealth.com. Personal information submitted by job applicants may be retained for as long as 3 years. In the event that the applicant is employed, that information will become part of a larger employee data base which is maintained in accordance with the requirements of federal law. Personal information submitted by physicians inquiring about practice opportunities may be retained indefinitely, unless we are informed that the physician has located an opportunity elsewhere.
Protecting the Privacy of Children
Horizon Health does not market to or employ children. Children should always get permission from their parents or guardians before sending any information about themselves (such as their names, email addresses, and phone numbers) over the Internet, to us or to anyone else. If you’re under 18, please don’t provide any personally identifying information during your visit to horizonhealth.com.
If you provide us your email address, from time to time we may send you emails about job opportunities with our company that we believe will interest you. We will not share your email address with providers of products or services. If you do not wish to receive email about job opportunities, please indicate when you give us your email address that you’d prefer not to receive such information. We may also use your email address to answer any email you send us, or to follow up on your visit to horizonhealth.com. Such follow-ups may include inquiries on transactions or submissions of information that appear incomplete.
Information Collected by Third Parties
Disclosure of Information
The personal information supplied by applicants for employment or persons interested in physician practice opportunities is shared selectively with individuals in the Human Resources Department of Horizon Health as well as recruiting and hiring managers in our home office and at our affiliated hospitals who have a business reason to know of the information because of their assigned duties. Additionally, we may occasionally release information about our visitors when release is appropriate to comply with law, or to protect the rights, property or safety of visitors to our site, our customers, the public or Horizon Health. As our business grows, we may buy or sell various assets. In the unlikely event that Horizon Health, one of its affiliated units, or substantially all of its assets are acquired by another company, information on our visitors would be among the transferred assets.
Accessing Your Registration Profile
We maintain registration profiles only for physicians who have completed the New Candidate Registration. To change any of the personal information appearing in your profile, visit My Profile, and edit the information appearing on that page of this web site. You will need your Username and Password to make changes to your profile. If you’ve forgotten your Username and Password, you can retrieve it online by submitting the e-mail address you previously provided to us.
If you have applied online for a position, you may change any of your personal contact information by calling, writing, or e-mailing the Corporate Recruiter whose contact information appears on the Contact Horizon Health Employment page of this web site.
Browser Level Information
Our Web server automatically collects your IP address when you request pages from our server. Your IP address is a number that is used by computers connected to the Internet to identify your computer so that data (such as the Web pages you request) can be sent to you. Your IP address does not tell us who you are. We also collect information on the operating system you’re running on your computer and the Web browser software you’re using to access our site so we can optimize our site for the most popular operating systems and Web browsers.
